Appointment Request
- To schedule, click on your choice of date and time in the box directly below and then fill in the requests for information.
- After scheduling you will receive an email confirmation. Please click on 'confirm' so that it registers on my schedule. If you do not confirm your appointment, your session will not show on my schedule.
- If you cancel your appointment on the day of your appointment, and are unable to reschedule for that same week, you will be charged for that appointment.

PAYMENT INFORMATION
Method of payment in order of preference - Zelle, Cash, PayPal, Credit, or Check
Individual & Couple sessions - $150.00 per 45 minutes. I do not offer a "sliding scale." I keep my hourly rate on the low end of what most therapists charge.
Evaluation - $800.00 (see Evaluation page)
The Good Guy/Gal Club - $30 per meeting following meeting with me individually ($150.00)
Anger Management - If you're interest in Anger Management is related to a legal issue, the courts typically require Anger Management 1 or 2. Anger Management 1 is 8 hours / 4 evening sessions ($200) following meeting with me for a 1:1 ($150). Anger Management 2 is 16 hours / 8 evening sessions ($400) following meeting with me for a 1:1 ($150). For alternative requirements, each 2-hour session is $40.
Urine Analysis - $40.00 for a 6 panel to include ETG
Regarding insurance, while I will provide you with an invoice for moneys paid. I require payment at the time of service.
Legal Paperwork: All legal paperwork (with the exception of assessments/evaluations) are included with services provided at the $150.00 per session rate.
